ALL  STUDENTS  MUST  HAVE  THE  FOLLOWING  MATERIALS  WITH  THEM  FOR  THEIR  REGISTRATION  APPOINTMENT  BEFORE  ANY  PAPERWORK  OR  SCHEDULING  IS  COMPLETED:

  • PROOF OF RESIDENCY -
    This may be either a Lease Agreement or a Settlement Statement which shows your name and physical address, AND a current utility bill (Natural gas, electric or water ONLY) in your name which shows your service address in our district. If you are temporarily living with someone in our school district, you must have a residency affidavit signed and provide proof of his or her residence.
     
  • WITHDRAWAL FORMS/TRANSCRIPT FROM PREVIOUS SCHOOL

  • DISCIPLINARY RECORDS -
    Per State Law O.C.G.A 20-2-67, new students must provide disciplinary records at time of enrollment.

  • GEORGIA IMMUNIZATION RECORDS -
    A current, up to date, Immunization Certificate must be presented at time of enrollment. All immunization records must be on Georgia Form 3231 (this may be obtained through the County Health Dept. or your own Georgia physician). THERE ARE NO EXCEPTIONS.

  • VISION, HEARING AND DENTAL CERTIFICATE -
    Required for all students entering a Georgia Public school for the first time.

  • STUDENT MUST BE ACCOMPANIED BY PARENT/GUARDIAN -
    Legal Guardianship must be verified with a Gwinnett County Probate Court document which assigns custodial duties to a person living in our school district. Phone number to Probate Court is 770-822-8266.

  • SOCIAL SECURITY NUMBER -
    required for Hope Scholarship eligibility.

STUDENT PLACEMENT INTO COLLINS HILL HIGH SCHOOL WILL NOT BE FINALIZED UNTIL THERE IS CONFIRMATION THAT NO DISCIPLINARY ACTION IS PENDING AT  THE  STUDENT'S PREVIOUS SCHOOL.



UPON ENTRY, GRADE PLACEMENT WILL BE BASED UPON RECORDS PROVIDED BY STUDENT. PERMANENT ASSIGNMENT WILL BE DETERMINED UPON RECEIPT AND EVALUATION OF TRANSCRIPT.



NOTE: If a student enters after the fifteenth (15) day from the beginning of the school year or the beginning of 2nd semester, no work can be made up and no credit awarded unless the student has been attending another accredited school and has transfer grades.
